Provides education and training to help individuals live more independently within the community. Provides hands on assistance with reminding, guiding, eating, bathing, dressing, grooming, personal hygiene, dusting, vacuuming, laundry, grocery shopping, meal planning and perparation, or any other task that an individual would do for themselvs, if able. Service occurs in the home or the community, according to need.
Enables an individual to live in their own home with a non-related person who is not legally responsible for the individual. Services provided in homes owned or rented by Peak. Staff will provide assistance with daily living skills, meal planning and preparation, home care and cleaning and medication management and arranging for transportation. Staff may provide overnight supervision. Individuals pay a portion of the cost of room and board.
Provides physician ordered therapy, including occupational, physical and speech therapy. Therapy will be provided by a licensed Therapist and shall include an evaluation of an individual's level of functioning by applying diagnostic and prognostic tests. Services shall be ordered by a physician in a specified amout and duration to guide the recipient in the use of therapeutic, creative, and self-care activities to assist the recipient in obtaining the highest possible level of functioning and shall be exclusive of maintenance or the prevention of regression and will include training of other providers on improving the level of functioning.
Provide short term care for the relief of the primary caregiver.
